Doctors, physical therapists and medical professionals all agree that the trick to any tendon injury is getting it to heal with minimal scar tissue formation and with as much realignment of tendon fibres as possible.

Even with optimum healing, there's always less elasticity in a previously injured tendon. This will cause the tendon to hurt during exercise and everyday activities. However, if you heal your injury efficiently and quickly with conservative treatment methods, your chance of re-injury later on is much lower.

Most doctors will recommend conservative treatment options to heal the torn tendon tissues as a minimally invasive approach. Other approaches (like surgery) are known to introduce even more scar tissue into your injury, and so this is why doctors will always recommend conservative treatment methods first as a "try everything" approach. The more diligent you are with your treatment and rehabilitation, the faster you will see successful results.

It is generally understood by doctors and surgeons, that surgery will introduce more scar tissue into the tendon and surrounding soft tissue. This added scar tissue will be problematic, requiring physical therapy and conservative treatment options post-surgery. If not dealt with properly, you could end up in worse condition than before the surgery. This is why surgery is only performed as a last resort.

Most doctors, physicians and orthopedic specialists will recommend conservative therapy for minor soft tissue injuries before considering surgery.

Some conservative treatment methods recommended for tendonitis include:

  • Rest - This is important for initial healing to reduce pain, swelling and inflammation in the early stage of your tendon injury. Too much rest can also be harmful because joint immobility can actually cause stiffening in the joint. This is why rest should be used when reducing initial pain and swelling, but should not be considered for more long-term conservative treatment.
  • Avoid Activities that Caused Your Injury - While resting your injury it's also important to avoid all activities that may have caused your tendonitis (especially if you now have tendonitis from overuse or repetitive movements). Continuing on with regular activities will not only make your injury worse, but trying to 'work around' your injury will eventually give rise to over-compensation injuries in other areas of your body.
  • Use a Cold Compress or Ice Pack - Cold is very effective at reducing pain and inflammation - use at the onset of the injury and during flareups.
  • Use Deep Heat Once The Swelling is Down - After any inflammation and/or swelling has been reduced, you can use your own blood flow to maximize healing of damaged tissue and maintain healthy blood flow to your tendons and surrounding soft tissue. Promoting blood flow to your tendon and surrounding soft tissue will help minimize the growth of scar tissue, increase flexibility and help prevent atrophy.
  • Avoid Over Compensating for your Injury on your "'Healthy' (Non-Injured) Side - Many people will start limping or carrying objects with their opposite arm to compensate for their tendon injury. Our bodies can adapt easily to any changes, including a tendonitis injury. This quick adaptation could mean that you're already compensating for your injury without even knowing it!

    When you compensate for your injury by using your opposite ('healthy') arm, leg, shoulder, knee, foot, etc. - then you are putting more weight and pressure on that side of your body. In many cases, your dominant side was injured (if you're right-handed this would be your right side) so your weaker side is trying to pick up the slack! The pain, stiffness, swelling and inflammation you then get on your 'healthy' side is something referred to as 'over-compensation pain'. Over-compensating for your injury can lead to other injuries and so that's why over compensation should be avoided at all costs OR treated with the same conservative treatment methods as your tendon injury.
  • Stretching - Stretching your muscles and tendons in physical therapy and at home will help you to regain your range of motion much faster than not stretching at all. Stretching in many ways is key to maintaining good Range of Motion (ROM), and stretching can be made much easier with the use of a TShellz Wrap before to warm up soft tissue, and a Cold Compress or Ice Pack treatment after to deal with flarups of swelling and inflammation.

It's important to rest the torn tendon because our natural healing process takes time to heal completely. If you don't rest your tendon injury, your acute tendinitis can quickly turn into a chronic tendinitis injury (tendinosis, tenosynovitis, calcific tendonitis, etc). Scar tissue may plague you for weeks, months and maybe even years, depending on your level of activity and the amount of conservative treatments you have done during your rehabilitation. Scar tissue is a major problem, especially when it comes to re-injury of your tendon. When dealing with scar tissue it's always important to:

  • listen well to your physician and if conservative treatments are recommended, remember to stick to your (daily) treatment plan using these therapies.

What About Surgery?

Before surgery is even considered, your physician will advise many months commitment to a conservative therapy program at least 6 consecutive months. Why so long? Supports and Exercises

Your doctor may encourage you to use a cuff or brace. The brace or cuff typically adds light compression to the area to support the tendons. Most physicians caution you in not wearing a brace too frequently, as there is often restricted blood flow and the soft tissue is then not allowed the time to heal. In some cases, prolonged use of a brace may cause increased secondary conditons such as pinched nerves and interruption to the blood flow to the joint.



Restoring Flexibility and Strength

Physical Therapy is a very helpful way to decrease pain in the soft tissue, restore strength to atrophied muscles and improve strength and mobility. Your physical therapist will start you on a program will be tailored for your tendon injury.

Once your pain starts to reduce, a physical therapist will also set up an individualized strengthening and stretching exercise program for you to perform at home or in the gym.

This will be based on your needs and abilities, and will help you return to performing your normal routines.

A trained physical therapist will help to ensure your rehabilitation process is effective. You may also need to change equipment you use at your job or at home to help you perform tasks more effectively and safely. You may opt to start taking a taxi temporarily. More frequent short breaks (and even stretches) at work are usually very helpful, as this allows your body a chance to relax and take some strain off your stressed soft tissue.

It is important to speak with a medical professional or occupational therapist to get more information about proper equipment, procedures and tools necessary for the specific activity you participate in or the office/work setting you work in during your normal daily life.

Sometimes this involves modifying or limiting the way you participate in activities.

Dealing With Negative Emotions and Planning for Success

It is important to keep a positive attitude, but sometimes that can be difficult. Focus on your goals and create a realistic plan on how to achieve them. Okay, So I Have a Tendinitis Injury...What Should I Do Now?

Stage 1: If the injury is very recent - use a good quality cold pack and Arnica Pain Relief Cream.

Note: If you are suffering from a tendinosis injury that has been going on for awhile, then skip the cold treatment as inflammation will be minimal. Anytime you are experiencing significant pain, you may find our Arnica Infusion Pain Relief Cream to be quite helpful.

If you are experiencing pain and swelling in the very early stages of the injury (first 48 to 72 hours), the first step in a conservative treatment protocol would be to focus on reducing the discomfort by applying a quality cold pack to the injured area along with a high quality pain relief cream such as our Arnica Infusion Cream.

Not only will you use your cold pack for the first 48 to 72 hours after the injury, but you should also use it during other stages of the healing process. Some examples include...

  • after any activity that stresses the joint in a moderate way. It is wise to quickly apply cold therapy to not only reduce the pain, but to also minimize tissue damage that may have occurred during the prior event.
  • If you have a job that requires you to place some demands on your injured tendon. If that is the case, use cold therapy immediately following work and if the opportunity allows - use it for 15 to 20 minutes during lunch or another break time.
  • If you are in a post-operative situation where cold compression therapy would be utilized heavily for at least the first few weeks following the procedure. Similarly, if you are scheduled for upcoming surgery, then you will need a high quality cold therapy device to use.

Having a Cold Compress or Ice Pack available at home to use if and when the need arises is helpful. Many of you already do so.

However, too many people over-focus on cold therapy while ignoring the most critical aspect to healing - nourishing the injured tissues with a healthy supply of blood flow needed to repair and rebuild the injured tissues.

This leads to the most important recommendation...

Stage 2: Focus on Increasing Blood Flow To The Injured Tissue - As This is The ONLY Way It Will Heal For the Long-Term

Even though the concept is simple, improving blood flow to injured tissues can be difficult. When the injury is tendon related, the challenges are even greater as the tendon receives very little blood flow. Traditional methods require your muscle to move to promote blood flow (exercise), but that same motion that promotes blood flow can at times lead to making your pain and condition worse.

  • Have you reinjured your tendon by returning to work too quickly?
  • How many times in the past have you aggravated your tendonitis just trying to perform basic tasks around the home?

Most people we deal with tell us these scenarios have happened to them many times in the past. Perhaps it has already happened to you.

Promoting blood flow within a muscle or soft tissue injury to help the body heal itself is a concept that has been utilized for centuries. This is where the focus has to be if you are seeking long-term improvement.

Oxygen and nutrients, carried within the blood, are critical for the body to heal itself. Without proper blood flow, recovering from an injury or condition will be delayed...sometimes for a very long period of time.

The real challenge is how do you promote blood flow to the area without causing further injury? This goal is further complicated by the fact that tendons are tough dense tissues that receive very little blood flow.

When people think blood flow beyond exercise, they instantly think "heating pad". Unfortunately, many individuals make the mistake in believing that a simple heating pad or microwavable gel packs will provide the blood flow needed to heal injured tissues. Nothing could be further from the truth.

To begin with, heating pads and warmed gel packs work on conduction. The heat they generate has to be forced through multiple layers of skin and fatty tissues. With each layer of skin the heat passes through, there is a corresponding energy loss because that heat is absorbing into each layer it passes through- the skin is an excellent insulator! Your body responds to this heat by increasing the blood flow near your skins surface (not in your injured tissue) to dissipate the heat and it does so quite effectively.

By the time what little heat reaches below the skin, sub-dermal layer and fat layer into the targeted area (the dense, injured tissues), very little energy remains and this does not effectively enhance blood flow. With little blood flow reaching these injured tissues - healing just doesn't happen.

The heating pad may feel comforting when on the body, but that is because the warmth is absorbing into the skin. Moist heat has been believed to be more effective at warming internal tissues versus dry heat, because it is believed that water transfers heat more quickly than through air. Clinical studies do not support that popular belief.

When you are using a heating pad, all that you are really accomplishing is warming of the skin. The unintended consequence is that you draw blood flow AWAY from the targeted tissues (the injured ones deeper in the body) and direct the blood flow to the surface of the body.

Warming the skin will not significantly contribute to healing of muscles, tendons, ligaments, cartilage, or any other tissues.

 

So If A Heating Pad Is Not The Answer, What Is?

In order to effectively stimulate blood flow to the dense tissue deep within the body, an energy based on an electromagnetic principal (diathermy) is needed. In essence, this is a conversion energy vs a conductive heat (like the heating pad example above - that is conductive heat).
